Sebuah rahasia besar terkuak di hari menjelang pernikahan Abi (Richard Kevin) dan Alya (Bunga Citra Lestari) dimana Abi menemukan buku harian milik Alya dan mengetahui bahwa Alya masih mencintai pria masa SMU nya, Sani (Ben Joshua). Kondisi Alya yang sedang koma karena kanker, membuat Abi berniat untuk menemukan Sani dan mempertemukannya dengan Alya Sani akhirnya berhasil Abi temukan dan dengan berat hati, istri Sani, Sisi, memperbolehkan Sani menjenguk Alya. Sani terharu melihat kondisi Alya yang dahulu lincah dan tomboy, nampak terbaring lemah tak berdaya. Akankah perasaan Sani berubah pada Alya? Bagaimanakah akhir kisah ini?
